About the Position
The Community Treatment Order (CTO) Coordinator provides consultation to community agencies and hospitals, coordinates services for patients who are appropriate for or subject to CTOs and develops partnerships within the mental health system to provide education, support, advice, and leadership on the use of Community Treatment Orders.
Key Responsibilities
- Lead and organize, in coordination/cooperation with physicians and treatment teams, a comprehensive individualized plan of care for patients identified as appropriate for Community Treatment Orders (CTOs), including preparation of paperwork, case conferences, liaising with treatment providers, community case managers and resources, and providing support to patients and families.
- Develop and maintain working relationships with hospitals, community service providers and programs to assist with Community Treatment Orders and discharge planning into the community.
- Monitor and evaluate information regarding the usage of Community Treatment Orders, gaps in the system related to CTO development, and the identification of program and service needs. Report findings to appropriate authorities.
- Develop a close liaison with community Mental Health Case Managers to ensure regular communication amongst the treatment team.
- Provide ongoing training and education to hospitals and the community regarding Community Treatment Orders.
- Participate in and lead research, program evaluation, teaching and education of Social Work students and support students from health-related disciplines.
- Keep current of hospital policies and procedures, applicable legislation and professional standards by reviewing current literature, attending educational events and networking with colleagues.
- Provide educational presentations to community groups.
- Required to travel within the Champlain Health District.
Qualifications
- Master’s degree in social work from an accredited university.
- Current registration and member in good standing with the Ontario College of Social Workers & Social Service Workers.
- Knowledge of the Mental Health system, the Mental Health Act, the role of the Public Guardian and Trustee’s Office, the role of the Rights Advisor and other related Mental Health Practitioners. Knowledge of the Psychiatric Advocate Office, Patient representatives, Health Care Consent Act, Substitute Decision Act.
- Minimum 3 to 5 years clinical psychiatric experience in the community and in a Mental Health Care or Chronic Care setting.
Language Requirements
English level A- is mandatory in oral expression, comprehension, reading and writing. Bilingual (French/English) is considered an asset
